• Detailed work history for the past 15 years, including job titles, dates, and duties
• W-2 forms or self-employment tax returns for the most recent year
• Past pay stubs or income records if applying for SSI (which is income-based)

Medical Evidence and Records

This is the most crucial part of your disability application. You’ll need:

• Names, addresses, and contact info for all doctors, clinics, and hospitals you've visited
• Dates of medical visits
• Diagnosis reports and treatment plans
• Lab test results, imaging (MRIs, X-rays, etc.), and surgical notes
• Prescription history
• Statements from treating physicians outlining your functional limitations
• Mental health records (if your disability is psychiatric or psychological)

SSA may request this information directly, but it’s better to provide as much as you can upfront to strengthen your case.

• Education and training history
• Workers' compensation records (if applicable)
• Military discharge papers (Form DD-214) if you served in the military
• Bank account information (for direct deposit if approved)
